Medical professionals have been utilizing lasers in their practices since the 1960s. When used for dermatological uses, different wavelengths target different cells in the skin. Laser hair removal operates through the process of “selective photothermolysis”. This term refers to pulses of laser energy that are selectively absorbed into the targeted tissue components known as chromophores. When targeting hair follicles, the laser is adjusted to a wavelength that focuses on melanin (around 300 to 1200 nm).[1] When the melanin in the hair shaft absorbs the low-level laser energy, it spreads it out to the surrounding hair structures like the hair matrix, where the hair is formed. The heat of the laser energy diffuses from the melanin cells to the follicular structures and destroys them.[2] Once destroyed, hair regeneration is hampered. This is what stops future hair growth in the treated areas.

Traditionally, other laser devices are not well-suited for darker skin types and can overheat the target tissue, resulting in burns. With these two hurdles in mind, Cutera (the makers of Excel HR) sought to construct a laser device that is safe on all skin types and doesn’t damage the skin in the process.
When melanin absorbs the laser energy, it is destroyed. For patients with darker skin types and more melanin, this can result in white spots, or hypopigmentation, in the treatment area. The Excel HR laser uses both Nd:YAG and Alexandrite lasers to accomodate all skin types. The Nd:YAG operates at a lower, longer wavelength of 1064nm. This wavelength is just enough to target and absorb deep follicular cells, while allowing the pigmented melanin cells to remain intact.
Once the laser energy spreads to a depth between 5-7 millimeters it hampers the follicular cell production, keeping them from regenerating. Patients with darker skin types see a significant reduction in hair growth without the dreaded white dots plaguing the results of other dermatological lasers.[3] In addition to the Nd:YAG laser, the Excel HR laser can switch to an Alexandrite laser: a laser that operates at 755 nm and is ideal for lighter hair.[1] Because of its dual setting lasers, the Excel HR laser is best at treating the entire spectrum of skin and hair types.
Attached to the Excel HR laser applicator is a specially designed sapphire cooling addition to ensure patient comfort during their procedure. With other laser hair removal devices, the thermal energy is difficult to regulate, commonly resulting in burns or severe skin irritation.
When the dermal tissue is exposed to prolonged levels of heat, it can cause considerable pain and damage to the target tissue. However, with its sapphire cooling addition, the Excel HR laser can better regulate its thermal effect on the skin and cool down tissue before it reaches uncomfortable temperatures. While the Excel HR’s sapphire cooling system effectively minimizes pain, we will begin your procedure with a topical anesthetic to alleviate the risk of any additional discomfort. We will ask and see that you followed the preparation instructions accurately. Then, we will have you lie on your back or stomach and target the laser on the shaved area. The laser will emit pulsed energy into the skin. Some patients liken this feeling to the feeling of a warm rubber band being flicked on their skin. We will do one pass over the follicles. For patients with more compact follicles, we might do one or two more passes. After your Excel HR laser hair removal, the treated area(s) will be red and tender. It will feel a little like a sunburn. Be sure to continue avoiding topical products that cause photosensitivity. When you go outside, keep your treated areas protected with covering or broad-spectrum sunscreen. The area(s) will be red and tender for the next couple of days.
